I don't know if this is true with all insurance companies, but I do know that at my HMO, folks were approved for WLS and because of the backlog of patients waiting to be enrolled in the pre-op classes, many people found that their insurance coverage changed because it went from one calendar year to the next. There were several people in my surgery class who were worried that they would not be covered for WLS because they received letters stating that their HMO coverage no longer included a WLS rider. It turns out that whatever their coverage said at the time they were actually approved for WLS, that's what the HMO provided even if their coverage had changed by the surgery date.
So (long story not so short), perhaps you will have a similar outcome because you have already been approved for the surgery. They may have no choice but to grandfather your surgery and cover it completely, regardless of how your hearing turns out. This might be something to explore.
