A HUGE Dr. Kemmeter fan here, and I looked into a lot of different options pre-op. He is a very skilled surgeon, and I felt completely at ease in his hands. If you would like to know why I decided upon Kemmeter as opposed to Foote, just send me a PM.
Now, GHP is a whole other story! Take it from me, they don't know squat about what your post-op DS needs will be. They only know how to provide guidance to someone with a RnY -- so stick around here for your best advice on what you true needs will be. Especially supplementation!! It is essential to your health that you NOT listen to them.
Good luck jumping through all of the insurance hoops. I love my post-op DS life, and can tell you that all you are going through right now to make it happen will absolutely be worth it.

Highest Known Weight = 370# / 59.7 bmi @ 5'6"
Current Weight = 168# / 26.4 bmi : fluctuates 5# either way @ 5'7" / more than 90% EWL
Normal BMI (24.9) = 159#: would have to compromise my muscle mass to get here without plastics, so this is not a goal.
