What you can do if your doctor won't order all the labs you need

What does the lab slip itself look like? How do you get a copy to write the labs on? Do you just type up a blank document with your personal info, the labs to be drawn, your dr's info & a line for him to sign? tx!

I just type it up.

I took a lab slip and sort of copied it.

You need the doctor info. Your info. A dx code(s). And the lab list.

I'm not even sure that it has to be signed, but I get it signed.

Thanks, Becky! When you wrote earlier 579.9, did you mean 579.8 (intest. malab.) on that list or is 579.9 something else?

I asked the forum before I wrote my last lab slip and Dr Steve (and others) advised me. I *think* it was .9 but I could be wrong.

someone else posted here after that about how the 579.9 is wrong so research that a little more too . . .

I think I remember that post, but I think it was more of a warning that some ins companies won't cover under that code, but some do. Trial & error, I guess, depending on your co.

Good advice. I'm thankful my PCP will order anything I want, whenever I want it. He's great like that. And he's willing to learn about the DS and tends to know quite a bit already.
