I think I can find a copy of my letter to insurance and a copy of my Dr's letter of reccomendation for the surgery.  If you have a place I can fax them to e-mail the number.  I do have a scanner at home now but havne't tried to use it yet.  I could also mail you a copy if you don't have a fax.  I'm off the rest of today and tomorrow so Monday would be as soon as I could fax it.  If you mean you need diagnosis codes to attach to the list of comorbidities I can get those for you also if you send me a list of what they are.     Jan [email protected]
