siho/lutheran preferred approval for plastic surger

I am looking for someone that has had any kind of plastic surgery and how and what you needed to do in order to get approval from insurance.  I am from Columbia City area and seeing a plastic surgeon in Fort Wayne. Thanks for anyone that can help with comments on insurance and plastic surgery.

I recently had a tummy tuck with full abdo and an inner thigh lift. The insurance covered the tummy tuck portion. The remaining ~$9,000 was out of pocket. The insurance required that I had a history of rashes from the apron of skin. I don't know if they had other requirements or not. If they did, I apparently met them. The usual requirements which may be wanted is that you are at least 12-18 months post-op from WLS and/or have maintained the weight loss for at least 1 year. Both of these, I had done...so I'm not sure if my insurance had required them or not. I had only went to the doctor once for the skin rashes....apparently that was enough times for my insurance.
