My surgeon's office reported someone else's information on my pre-determination!

I found out that my surgeon's office recorded someone else's information in my chart and reported incorrect information for me to my insurance company.  I went back into my surgeon's office where they re-weighed me so they could submit the correct information to my insurance. My insurance company denied me because of this incorrect information.  Now I don't know whether to send in appeal to my insurance company or just have my surgeon's office re-submit my information as a new pre-determination.  Any suggestions?  I'm afraid that my insurance company won't believe me that the surgeon's office submitted information of someone else.  I don't want them to automatically deny me again because they think I'm just re-submitting information (BMI) that DOES qualify for coverage. Any opinions?
