Plastic Surgery Insurance Template

I am going to have an abdomonoplasty done in December and know that my insureance can pick up some of the costs...I am going to write a great letter for my PS as he does not accept insurance but is willing to do the letter and then I do self pay and get reimbursed from insurance. Does anyone have any template letters? Thanks for your help, Ann
