Advice writing appeal

Hello! I was recently denied a request for breast reduction surgery.  I'm in the process of writing an appeal but I need some suggestions on how to organize the letter.  I would appreciate any assistance you are able to give me.  The insurance gave the normal spill that it is an elective surgery for cosmetic reasons but I've lost more than a hundred pounds and my breast are hanging to my waist and I also have skin rashes.  I appreciate you listening.  Thanks! I recall hearing that there is a certain amount of tissue that needs to be removed from each breast in order to be a medical necessity. The removal of breast tissue needs to be to "Imporve a physiological function." ---I can't recall how much tissue (the estimated weight of the tissue) that is needed to qualify for 'necessity.' Your Md would submit that information with your appeal. Also, well documented skin breakdown and infections (to show high risk) and any shoulder 'grooving' -bra marks/damage from heavy breasts..... documented back pain, if you have that. Basically, it is almost always 'cosmetic' unless it 'improves a physiological' function.  Best of Luck!

Don't give up on that one!  I had it done 1 year ago and would do it again 100 times.  I was approved right away and they took about 6 lbs of ??fat  I guess.  Try working with your doctors, I had referals from my ob, and chiropractor, and pcp.  Good luck.  It is much more than cosmetic!
