I spoke with a lady at my insurance company on Monday, and she confirmed that WLS is covered under our policy. She told me that I would need to submit a letter of medical neccessity from my PCP, a letter from the Surgeon, along with a psych eval and a NUT consult before a decision will be made as to approval or denial.
I asked the lady about the BMI, and she had no clue as to where I could find out the answer. All I want to know is how high/low does the BMI need to be on order to qualify. I called several numbers associated with my policy, and no one knows the answer.
I tried to find a website with the info, but I come up empty handed and its not covered in my policy book.
I keep reading that a BMI of 40 or more is the "magic number" so to speak with the majority of the insurance companies...am I just getting nervous for no reason?
Has anyone dealt with Freedom Network~Construction Workers Laborers Welfare Fund before that would know the answer to the BMI question? How was the experience for you?
