Anyone w/ Activa Benefits in Gr. Rapids ins.??

Think I may have originally posted this on the wrong board, sorry!  Still new at this posting stuff.  Hello, I'm wrapping up my 6 mos. supervised diet w/ my pcp in May and I was wondering if anyone out there has had experience w/ Activa Benefits out of Grand Rapids, MI? They had a whole list of criteria I have to meet. Co-morbidities, last 5 yrs. of records indicating that my weight has been significant, high BMI of 40 or more, etc., exercise, (joined a gym and work out regularly, does this count?, also saw a personal trainer about 4 times to get me started??), etc. The question I guess is are they difficult to get approval from? In anyone's experience? Also, I have to meet EACH one of the following criteria, there's a list, I will only ask for advice on 2. (1) was supervised & monitored by a physician working in cooperation w/ dietician and/or nutritionists & documented in medical records by a physician who does not perform the bariatric surgery. (2) Behavior modification or behavioral health intervention. I didn't see a nutritionist or dietician I just followed WW program and my Dr.s advice on the South Beach diet plans. Does this count? As far as behavior modification what are they looking for here? I attend TOPS meetings regularly and we discuss ways to eat and exercise and live a more healthy lifestyle?? Sorry to drag on, I'm just concerned that they are going to try and deny and I'll have to appeal, blah, blah, and I will if I have to. Just curious if anyone's had experience w/ them. Thanks so much for any advice or input in all this. I'm so ready for a new life and waiting has been so stressful! I want to live again, soon!

Hi there.  I also have Aetna, but the PPO.  The guidelines are still the same though.  I talked to the nurse at Aetna who does the review of the authorization request.  I asked her about the 3 month mutlidisciplinary thing and she said "You can't go through a doctor.  Doctor's don't know anything about diets!  You have to go to a dietician."  yes, she really said that.  She told me that my dietician and PCP had to have everything documented in my medical records.  So I'm going through a dietician and meeting with her on a weekly basis for now.  I also have to go to my PCP each month as well.   So I would call them and ask about what you are doing.  You will have a person specifically assigned to your member group. 

As far as behaviour health, did you every see a psychiartrist/physcologist?  My dietician said she could do all 3 (diet, exercise, and behavioral).  I wasn't required to do a psych eval by my doc, so I hope the program with the dietician is ok.  Did you keep track of what you ate and how much you exercised using any tracking logs?  That is what I am required to do for my program.  If you have those, then they should prove to them what behavior you had in the last 6 months. Good luck!  Please let me know what happens.  I don't really trust the insurance companies, and I work for one!

Hey Paula, Thanks for the response.  I think you and I have different ins. co.  Mine is Activa Benefits PPOM out of Gr. Rapids, and you say yours is Aetna, PPO.  My Dr. has advised me what to eat some, but found out through my P.T. at the gym he is a nutritionist as well, he had given me a diet to follow, which I did, but didn't realize he's a nutritionist also.  Hmmmm.......that sounds good to me.  Yes, I've docuemented every behavioral change and all exercise, as well as, every morsel that goes into my mouth since Sept. of '06.  I plan to bombard them w/ dietary & nutritional information about me.  Instead of 5 yrs docuementation, I plan to give them 7-8 yrs.  I realized I can provide that!  I'm leaving no stone unturned as to the game they want to play!  Of course, I'm not giving them extra info, only a ton of what they asked for.   I think they will approve me in the end, if not the beginning.  I did find one other person who has used this ins. through her husbands work & she says they're not too difficult as long as you give them what their asking for.  She had failed to give them a 6 mos diet history w/ pcp so she had to do that first.  Then they approved.  I'm very hopeful.  Wishing you all the best!
