Help with insurance

I was wondering if anyone knew how long insurance companies actually have to make a desicion? I have Anthem and was approved back in Nov.'06. I then recieved a letter two days later stating they needed more information, and wanted to know every doctor that I had seen in the last five years. I sent that in the next day. I have been paying for my insurance even though it is in "pending" since Dec. They will not even cover me to visit the doctor for a cold.  When I call them, they tell me that they will only talk to my doctor. When he calls they tell him, it is in review and will be 30 days. After waiting the 30 days they say it will be 45 to 60 days... I am just wondering how long they can stretch it out? It has been in the same pending review since December, well over the 120 days. Any help or suggestions would be GREATLY appriciated.
