BCBS IL Appeal letter
Good Day Everyone,
I was wondering if anyone could help me?!? I have been denied Gastric Bypass due to the fact that I did not notify them in time that I was going through the hoops. My ins plan quit covering as of 1/1/07 UNLESS I sent them letters.. which they are saying they did not receive them in time. SOOO.. now begins the appeals process. Can anyone who has written a appeal letter to them help me with any information I should put in the letter? Thanks so much!!
