I called my insurance co. to check and see if there was anything I needed to do prior  to pre-authorization for surgery and they said no...the only requirement for surgery is that it is Medically neccessary, which is a BMI of 40 and up.  So I don't know if that means if the Doc says I need it and my BMI is high enough  I'll get approved?? They said my policy stated nothing about the 6 month supervised diet or any thing, the docs office says' we'll see what happens when it is submitted.  Any one else heard anything like this??

Most policies are or should be like that.  Medically necessary and that's that. Sounds like you'll be ok.  Don't overthink it.  Just get your records just in case.

Be Very careful here. That was what I was told as well. I went 8 months waiting including just getting my records to prove I needed it due to medical reasons and have now gotten two appeals due to a clause that was put into my husband employer that I never knew was there and the insurance company never told me about. So find out what is in your employee handbook or Members certifcate of coverage. I do not want to see anyone use up to appeals like me not knowing about a certain way something was written in their policy
