HOW CAN YOU ADD A 5 YEAR DIET HISTORY???

I had WLS 2 yrs ago, and had no insurance problems then.  Now my husband is interested and h our new insurance "Healthlink Open AccessI" requires a 5 year documented diet history.  My husband rarely sees any Dr. except for his Orthopedic Dr who suggested he have WLS.  He has a bunch of weight loss attempts and keeps a food diary and weight history, but this is not in any record.  Is it possible to type this info up and submit it to the primary Dr he never sees, or to give it to the Ortho Dr who is encouraging him?  Would the insurance see this in the record and okay it? Anyone out there with some advise or experience here?

The 5 year requirement is usually just to confirm your weight/BMI each year.  This is different from the medically supervised weight loss/diet.  I'd confirm with the insurance company what they're looking for.  If they just need to confirm his weight each year then notes from any combination of doctors will work just fine.  They only need one record from each year.  Now if they require a 5 year supervised weight loss then that's just crazy.  Normally, the supervised diet is 3, 6 or 12 months long.  I had 5 years of medical records proving my weight for each year.  They didn't submit all of the notes, just one record from each year.  In addition to that I had to complete a 3 month medically supervised weight loss.  Hope this helps.
