Empire Blue

My spouse recently got this insurance thru work.  I called the number thru his employer and was told that they do cover LapBand.  So I asked what the requirements were.  She said that first I would need  a letter from the Bariatric Doc explaining why I would need the surgery and if it was medically necessary.  She said that once the review board decided if it was medically necessary I would be given  case number then I could find out what the requirements were if any.  Has anybody heard of that.  My last insurance I was given the requirements once completed the packet would have been submitted for insurance.  If anyone has this insurance or knows I would appreciate it.

I have Empire Blue too.  I found their medical policy by going to their website www.empireblue.com.  Click on the provider tab like you are the doctor and on the right side of the screen you will see Medical Policies.  Look under Surgery for Obesity and it will allow you to bring up their requirements.  I don't know though about your specific policy,

Thanks for the link. Just keep in mind  nothing is controlling until you see it written in the policy.  So don't get excited or dissapointed until you see exactly what is covered.
