Aetna- 3 month approval- exercise documentation?
What I did was during my monthly meeting with my PCP for the three months I create a little summary page that included my exercise regimen (walk 3 days, workout 4 days). Then I would take it to him and he would sign it and submitted it with my paperwork for approval.

My surgeon and I would discuss my exercise routine during my monthly weigh ins and she would record what activities I did , how often , and for how long In my monthly progress notes. Then she would make suggestions on what she felt I should be doing to increase the activities. That way when I went back the next month we could show progress. The notes were dated and signed by the surgeon and included in the paperwork sent to the insurance company.
I went to a dietician who was able to take care of the diet, exercise and behavioral pieces of those requirements. Basically. I kept a food and exercise log for each day I was on the 3 month thing. Then she used those to compile my progress notes. I asked Aetna if they needed the food and excercise logs and they said that it wasn't necessary, but more info is better than less.
So I would provide those to your doctor or dietician tand ask them to specifically document in your progress notes that you are exercising regularly. Then when it comes time to fax your records, have them fax those as well. You should be covered just fine.
In my case, since my dietician had copious notes for my progress in diet, exercise, and behavor modification, she didn't need to send my food logs. I got approved in less than 24 hours!
Ok, on another note. What I have been suggesting to everyone is that you contact Aetna customer service and ask to speak to your nurse health advocate. This will be the person who will review your authorization request. I found it to be very helpful to get to know this person and build the relationship up front before they receive your actual paperwork. Also, ask this person what fax # to use when sending in the auth request. The reason is that there is a generic # that will take forever to get the paperwork routed to the right place. So why not fax it to the right person to begin with. This is probably the reason that I was able to get approval so fast.
