Limitations and Exclusions what do they mean????

I have Humana PPO of Illinois. I had gone to my 1st meeting about gastric bypass at the University of Chicago with Dr. Alverdy. The 2 days later i get a phone call stating that my insurance has denied me from even having the initial consultation with Dr. Alverdy, the nurses,dieticians and psych. I had my pcp and my endocronologist send a letter to my Insurance-Humana. I have received my denial letter on july 10 with this Limitations and Exclusions: Unless specifically stated otherwise,no benefits will be provided for, or on account of, the following items: 211200 05/05 Any treatment, including but not limited to surgical procedures: -for obesity, whi*****ludes morbid obesity; or -for obesity, whi*****ludes morbid obesity, for the purpose of treating a sickness or bodily injury caused by, complicated by, or exacerbated by the obesity. 215500 05/05 Does this mean it's over? I have no chance for appeal? Is this what Gary V says is Iron Clad? I have a BMI of 40 and co-morbidities. Any Info would be greatly appreciated from anyone. thanks Jeaniene
