Any idea what it means when a case is "closed"?

Being the impatient person that I am, I called my insurance company again today.  A couple days ago, I was in "medical review".  Today, I was told that my case had been CLOSED!  I started to freak out a bit thinkin this was another word for  "denial".  I was told that there was a note in the computer that said it was closed due to "unreadable documentation due to faxing" with my papers for my 6th month PCP supervised diet.  Some of the documentation was too light to read.   So does anyone know what "closed" means...does this mean I have to appeal to have it reopened or does it mean that it's closed until the papers are darkened and re-faxed?  Just to be sure, I copied the form from my supervised diet forms and typed them all out and resent each form with the text document as a reference.  From here with the originals, the paper is very readable and I could easily type what it said (thank goodness my PCP has good, readable handwriting). I'd send the originals but I'm afraid of them getting lost...you know that goes with insurance companies.  Anyway...any ideas or information is very much appreciated!

Thank you for your guidance and wisdom.  I do have that program and can save them as PDF files.  However, they state everything has to be faxed or mailed.  I'm confident they have email (doesn't EVERY office...especially one like a huge insuranc company!?!?).  I'll try to get that tomorrow.  In the mean time, I was able to darken them by copying them repeatedly (*thanks for the Kinkos tip*).  They're darker now that the originals.  I'm requesting they be sent along with the transcribed/typed version.  My surgon's office is really on the ball with this sort of thing.  I'll call on Tuesday to make sure it was received...but, hopefully, I"ll have an email address by then.  If not, I'll be sure to follow-up daily. Again, thank you for your wisdom on this board!
