Sample letter from PCP and/or surgeon for WLS?

Hi everyone, i just posted a thesis in the last thread, lol, about my recent denial for surgeon consult and I was just wondering if anyone had a sample letter to show me of what a PCP or surgeon might write to the insurance to help gain approval? I ask only because my PCP is the laziest and clearly doesn't like to do anything extra for this patients so I would like to show him a sample of what he could write for me. Also, I have never met with a surgeon because my insurance denied my request for a consultation/referral...so how could I get one from a surgeon? Can I pay for my own consultation with the surgeon and do you think they would give me one? I'm lost here...can somebody please help. Thank you!!!!

They're going to want a PCP letter,  very detailed, I saw Ms. X for X years and her treatments have been X, her diagnosis is X and she has "tried and failed multiple supervised low calorie diets:"  thats the key words. And they[re going to have to attach their chart notes as well.
