Anyone have BCBS of CA? Need Help

I have BCBS of CA and obesity surgery is covered.  My doctor gave me a pamphlet at the the seminar about the requirements of each insurance and these were the four listed for BCBS of CA:
1.  History of WL attempts
2.  Nutritionist visit
3.  Before and After program
4.  PCP referral

2 and 3 will be taken care of through my WLS doc, 4 shouldn't be a problem since he recommended it to me before but my old insurance didn't have the rider but I am worried about 1.  Does this mean I need physician supervised or will my attemps through weigh****chers, etc work?  Also, how do I know what types they cover?  I am interested in getting a band.

I have BCBS of CA and, like you, want to be banded. The insurance required 6 months of physician supervised, non-surgical weight loss attempts even though I've done Weigh****chers, Jenny Craig, LA Weightloss, etc. I completed the 6 months and now I'm waiting for the doctor to submit to insurance (I'm so excited ). Good luck and let me know how you make out.
