Central States BCBS Teamcare Please Contact Me

I am looking for others that have an insurance card of BCBS but has Central States in the left corner. My husband works for UPS and has the Teamsters Union which is a self insured insurance company. They are denying all claims as cosmetic. I know some of you have given up but I think we need to fight together. All self insured companys are not governed by the US Insurance Commission.
