The key is to call the care coordination department.  I called everyday customer service until one day they told me that I was calling the wrong department.  Once I started calling them, I was able to find out a lot more.  At two weeks you can request a status report.  They approved me in exactly 3 weeks and 1 day after they received my paperwork.
