What to do? - no referral from PCP

Get another doctor.  This "Little Tin Jesus," oh excuse me "doctor" is working against you not with you.  I would write him a letter and send a copy to his superiors and peer review committee.  This doctor is not interested in your health only the fact that he can dole out abuse to patients.  Don't become his victim.  Good luck.
