What does the information pack contain?

I got a call yesterday from HRH and the woman said she was sending out my "information pack" which needs to be completed before orientation is arranged. Can anyone tell me the content of this information pack?

I imagine that each centre is different, but from TWH I received a package with information about my orientation appointment and also a long questionnaire that needed to be filled out and returned before my orientation.

I went through Humber

The package includes an extensive personal history that you complete (basically a review of all body systems, weight loss trials in the past, what you're doing now to help yourself etc, I believe it was 18 pages long) and also a medical/physical form for your family physician to fill out, so make sure you get to your family physician before your orientation date. All forms must be completed and submitted in full prior to you being placed in the cue for future appointments

Referral date: May 19, 2016 | Info session: July 12, 2016 | Surgeon Approval: December 2, 2016 | Pre Op: January 24, 2017 | Trio: February 9, 2017 | Surgery Date: March 3, 2017, Humber River Regional Hospital

Starting Weight: 253 | Post-Optifast Weight/day of surgery: 237 | Current Weight: 123.2

Thank you! This really helps. I wasn't sure what it would contain.

My family Dr has a walk in every day so I can go see him at anytime. He's really good at making time and will be happy to help me with this. I'm moving "up north" early next month so I'm keen to get things in place before my file is moved to Sudbury to save me having to start all over again (which is what they told me).

So I received my information pack today. Are you more likely to be approved if you have depression or mental health issues? It kinda feels like you're supposed to have mental health issues and or depression for you to progress. I might be totally wrong or misguided in saying that but that's how it seems from the documentation.
