seriously, why can't docs understand the NSAIDS thing?

They take pharmacology in med school, right?  So why is it they don't seem to understand how NSAIDS work or why we can't take them?

Friday at ECT I mentioned how sore I was the last time and the anesthesiologist wanted to give me IV  Toradol to prevent it from happening that time.  She kept insisting it would be OK by IV.  I finally said "That is NOT what my bariatric surgeon tells me.  You may call her and discuss it with her, but you do NOT have my permission to administer any type of NSAIDS by IV or any other way without her approval."  I got Tylenol with  Codeine by  mouth instead - which worked just fine.

But honestly.  Wouldn't a basic pharmacology course explain how NSAIDS work?

I heard we should have one of those bracelets that say that we've had the surgery or no NSAIDS, I cant remember exactly.  I have yet to find someone with one on.

As a pharmacist, you are both correct. Ulcer risk increases with duration and dose of NSAID received. Short term toradol use is not likely to cause an ulcer whether it is administered iv or orally. Max is 5 days anyway due to renal problems.

A greater risk for ulcers is if you are h. Pylori positive. As with everything the more risk factors you have the more your risk increases. And as with any medication and or surgical intervention, it is always important to weigh the risks vs. benefits. Fortunately you had a suitable alternative.

My wls surgeon prescribed toradol post- op because I personally respond to NSAIDs better than codeine or hydrocodone. I also tested h pylori negative, do not smoke, and do not drink. Lower risk.

My doctor said no Prednisone either, and that's not an NSAID.

My doctor said it is becuase they are so trained now not to administer narcotics.  You could be a junkie seeking drugs.  NSAIDS aren't addictive and that is the protocol they are supposed to follow.  I agree it is annoying...everytime I am in the ER with a migraine it is the same story and I have taken torodal via IV several times because I have been given no other option. 

Luckily I haven't gotten an ulcer yet....but it is a gamble.  I almost want to get a letter from my doctor stating that I have a documented case of migraines and have had RNY and if I am in the ER they need to give me narcotics....not sure if she would write one or not.

I would ask for a letter.  She might not write one... but I bet my PCP or bariatric surgeon would, either one, if I was in that situation.  I'd ask.  Worst that can happen is she says no.

 My fiance is in medical school, and he said the exact same thing - it's pure protocol.
