Went for my monthly checkup with the surgeon and he said something that didnt dawn on my until I got home...

But that leads me to ask;

Do all RNY patients have the same "size" pouch?  or relativly close in size?  Im just curious.

If they are not the same size, how is it determined what size the pouch is?

No, not all pouches are the same.  One's specific anatomy determines size.

Some surgeons choose to make smaller or larger pouches.   Some may also vary pouch size based on person size... Tony (who is 6'8") was intentionally given a somewhat larger pouch than I was by the same surgeon because the surgeon knew that he would NEED to be able to eat more than I would (at under 5'4").

I was told by my doctor that they use anatomical guides to determine where the pouch is formed. The guides are visible veins on the surface of the stomach and the doctor counts them and at some point separates the stomach in between them. My pouch holds about 1oz. I think your anatomy determines exact size, based on what I've been told.
