I went every year for that!!! please help i really want to have surgery!!!
You'll likely have to check with your insurance company any find out if WW is OK with their wt loss guidelines. My insurance required a medically supervised wt loss program of at least 6 months duration with the previous 5 years. I had participated in such a private program that I paid for some 4 years earlier. I contacted them, and got a letter from the medical director of that program, and my insurance accepted that. Medical history from WW may not count, it depends on your insurance company. DAVE

I had a similar issue... no insurance and no medical records. Were you seen at any urgent care centers during that time? Get a sinus infection that you needed meds for? Sprained your ankle? Anything? Urgent care saved me. I had 2 visits at a local $40/visit clinic and they used those 2 records to track me.
Good Luck!

Yes, I had the same issue but in a different way. I had insurance, but my only visits over the 5 year period were for ophthalmology visits (eye doctors do not weigh) and my counseling sessions (and she did not weigh). Thus I did not have a five year history. As a result, I composed a letter explaining why I did not have the necessary documentation and requested it be included in my surgeon's documentation when it was forwarded to insurance for approval. In the letter, I also wrote a detailed history about my weight loss efforts naming every event, what I did to lose weight, exercise, etc. Most of my major life events have been connected to weight loss issues. I think it's that way for many of us. It took me about 10 hours to write the letter. I was thorough and knew how important this would mean for me getting approval because at the time of my first consult, my BMI was 34.7, and that is what was submitted. However my thyroid began failing and I gained additional weight by the time of surgery. I also like what another poster said about including photos of yourself..
