Not sure of one you could print out but I love myfitnesspal.com. They have a phone app too!

 I use http://nutrimirror.com/.  It allows you to add recipes and find nutritional info., log exercise, body stats., plus a lot more.  great group of people in their journal room.
Might want to look into it.

I use an empty check register! has all kinds of lines and boxes, and it fits into my purse very easily!

I like myfitnesspal, there is an app for my phone, and you can use the online version, they synch together.
It's easy to use, and I can log exercise too.
You can print the actual logs, summaries and charts.
