How many days were you in the hospital? I will also have my hiatal hernia repaired, insurance approved 3 days for me.

 1 day With hernia repair

I had to be at the hospital at 7am and was discharged the next day at 4:00pm.

just overnight with hernia repair too ( I didnt even know I had a hernia) lol was told after the operation that I had one.

 Two days for me. Bypass, large hernia and gall bladder.  

What happens if you need to stay in longer than 3 days?  Do you have to pay out of pocket?

 They adjust the approval by your doctor's orders. 

When I had my daughter, I thought I was going to have a regular birth. 27+ hours later, I had a c-section, and had to stay for four days instead of the normal two for vaginal delivery. My doctor had to let them know it was medically necessary for me to stay longer. 

Large hiatal hernia with the rny, two nights.

I was in two nights.  Discharged the morning of day 3.

 I stayed two nights, and was discharged around dinner time on the third day.
