Vitalady's bloodwork form....acceptable levels..... Can't find it!

I can't seem to find any of the bookmarks that I had on this site.  I'm looking for the form that Vitalady had that gave acceptable levels of all the vitamins and minerals when you had your bloodwork done.....are the bookmarks still here somewhere, but I am unable to locate them?  Thanks, in advance, for your input....
