How to add a surgeon?

I keep trying to add my surgery date and my surgeon doesn't show up. Every time I try to add him I get this really long error message and it won't let me do anything on my computer or my phone. Any ideas? Thanks! 

I had to switch to a different browser and then it let me add my date, surgeon, and a signature line.

I've been trying to add a surgeon over the last few days as I have had a revision to Roux en Y and won't let me add mine either. I have tried with Internet Explorer and Google Chrome and get a whole lot of code as the error message so gave up this morning and sent an email to the tech team to ask for help. If you send them one too might help to show that it isn't just one person.

Just tried with Mozilla and no success still.

Thanks ya'll. I just tried on Firefox, Chrome, and Internet Explorer with no luck. I will send an email too and see if we can get this solved. We're so excited about getting this done and now we can't even properly brag ;)
