Vitamin recommendation regimen for gastric bypass

There is an interesting article posted on OH that covers a vitamin regimen for post-RNY patients. I learned something new form it so I thought I'd pass it along for others who may be interested:

Hmmm.   The article doesn't mention the fact that patients with different types of WLS need different vitamins.  This article doesn't appear to be directed specifically at RNY folks but at WLS patients in general, but someone with a lap band has very different vitamin needs than someone with a DS (or even a RNY, for that matter).  She recommends taking additional vitamin A, vitamin E, and zinc, even though not all RNY patients need that.  In fact, a vitamin E deficiency seems to be pretty rare in RNY folks, at least as long as they are taking a good multi twice a day.  Vitamin A and zinc deficiencies are more common, but since it is possible to take too much of both of those, I personally would not suggest taking extra unless your labs show you need more or if you are taking a multi that is very low in one or both of those things.  The recommendations in that article look to me to be similar to what the ASMBS recommends for DS patients, not RNY patients.

I am pretty sure that she mentions this is for gastric bypass patients, but don't hold my feet to the fire on that. Can you share a vitamin regimen that you would recommend?

In the article, she says this is a "suggested program for bariatric post-op patients," which suggests lap band, RNY, VSG, or DS.

Anyway, I suggest the regimen suggested by the ASMBS, which for RNY folks is two multis a day, 1500 to 2000 mg calcium citrate, 54 to 63 mg iron for menstruating women and 36 mg iron for everyone else, and B12 (500 mcg sublingual daily, or weekly  nasal spray, or monthly injections).  I suggest adding additional vitamins/minerals and adjusting these doses based on your labs.

For the Calcium....I heard that it is  recommended that we take one every four hours (and of course not at the same time as iron).  Is this correct? Also, I take the bariatric advantage muti.....It states to chew one a day on the bottle, but should I be taking it twice a day? 

  Im only 3 weeks out and my surgeon keeps telling me not to worry about vitamins right now.  Which I think is weird, but  hes the doc not me =)   I meet with my Nut next week so I will be asking what she suggests.

At least two hours apart for calcium.  And we can absorb about 500 mg at a time.

I believe with the BA multi, we need two a day.  Check the label. Does on tablet have 100% of the RDA of things or is it 200%?  If it's 100% of most things, you need two a day.  If it has 200%, then just one a day.

Some docs suggest waiting a while to start vitamins but I think that's unwise.  The ASMBS says to start your multi and iron right away, and I think it's wise to follow their guidelines in most cases.

Thank you.  I just looked at my multi it is 100%. So I will start taking two.  I agree about the vitamin waiting...It just doesn't sound right to me.  I started when I got home from the hospital.  I dont need a deficiency.  Im going to look at the ASMBS website.  Thank you for the info.
