I am a therapist *****gularly does the pre-op psych eval. That is really really high. I mean, excessive. I would see if you can see someone else. You can expect to pay more than a standard hour of therapy because they have to write a report and have some special knowledge of WLS but $600 seems really high.
I also do pre-op psych evals and agree that $600 is excessive (no matter where you live (which does effect the cost)). You should be able to get one done for about half that much.

Since you are self pay, that is normal compared to what I paid, which was $500 or so. It all depends on what your insurance covers. Mine doesn't cover visits to Psychology professionals. Since you are self paying you will also have extra bills from the hospital and anesthesiologist that won't be included in the surgery cost. At least mine did.
I'm self pay also. Total cost for everything is estimated at $25,000. Everything pre op I had to pay as you go. the psych eval was $300. Then at the hospital check in the day of surgery, I paid $22,500. From this payment they cover everything I will need surgery, lab and physician wise for 90 days (it included a mandatory policy to cover complications from surgery).
