What documentation of previous diets do you need?
It really depends on what your insurance plan will accept. I had been on a ADA 2000 calorie a day diet for two years with Dr. Visits every 3 months. My insurance accepted that, even though it wasnt specifically for a weight loss program.

My insurance wanted proof too and all i had to do was print out copies of my bank statements showing that i paid for Diet items. then i had my gym print out copies of my attendance records. I also showed my doctor office weigh ins when i went in for appts. I also sent in my food logs that i kept while on my diets.

I had this discussion today with my insurance company (CIGNA) to get the scoop. Whatever weight program I go to I have to have a doctor who follows me and I meet with regularly (like once a month). My insurance wants doctor supervised documentation within at least a 6 month period. My bariatric doctor happens to have a class that I go to once a month. These classes are considered a doctor visit where I log my weight, bp, and BMI and monthly goals of things I will work on (exercise, cut out soda, caffeine etc).
My advice is to get on the phone with your insurance company and hear first hand what they require. Sometimes they want 6 or 7 classes once every 4 weeks. Some count your initial consultation as the starting point. Others don't. Some only want 3 months. They are all a little different so best to get it straight from the source.
