I subscribe to Maxx Benson's youtube channel and one of his videos mentions him getting free Bariatric Advantage supplements. I thought it was a hoax but I printed out the form, filled it out and gave it to my NUT to sign and fax in. Well lo and behold I got an email from Bariatric Advantage today saying my 180 vitamins, 270 calcium chews and 90 B-12 chews were on their way! Can't wait to get them.

Hi Dawn, I also live in MI and am pre-surgery mode. One week from today, I will have VSG. I am nervous, scared, and excited all at the same time. I just looked at the Bariatric Advantage website and don't see the form you are talking about. Could you share specifically where you found the form? I am always up for a good deal.
