Psych clearance letter examples

Hello everyone, I am in the process of going through my consultation and was told I need a clearance letter from my psychiatrist. The only thing is that he never did one before and need examples to go by. Does anyone who has been cleared have an example of a clearance letter for a psych evaluation or template that he can go by. I think this will be the only thing holding me back from getting the surgery. TIA ??
