He does require payment 2 weeks before surgery even with insurance reimbursement. As my insurance would not cover this procedure, I am paying for it myself, so I can understand the need for prepayment. But, if insurance is paying, it would seem that they could wait for payment from them.
He intensely believes in follow-up. He will see me annually for life.
