I'm in the process of getting my psych eval into my PCP's office for them to submit to the Pre-determination coordinator, who then submits to my Ins Co..
These are the things that my BCBS required in order to CONSIDERED for gastric bypass surgery
* 5 yr hx of weight, BP, etc. (My BMI has always been over 35+ since high school)
* Initial eval. (Got this w/ my PCP Dr.)
* Comordities along with Morbid Obesity (Doc. High BP w/ meds, Current BMI 52.4, asthma, family hx of diabetes, asthma, high BP and morbid obesity)
* 6 Mo. of Medically Supervised Wt. Loss Efforts (I went ev. mo. to get weighted in w/ my PCP and recorded any wt. loss or gain. Loss 8 lbs in 6mos.
* Psych. Eval. - Still waiting for them to submit to my PCP
* Two Forms stating that I'm really for all the lifestyle and Program changes.
* Nutrition Therapy & Exercise (I exercise from time 2 time but I only had to see the nutritionist once)
Its now April 4, 2008 and I'm truly hoping for approval. I hope that my entry may help someone else if they don't know what's required for approval. I hope I've done enough and shown my efforts as well as my dire need for surgery. If you know something that could help me out. Holla Back.

I recieved a phone call from my dr's office and they stated that I was approved for surgery. Despite the issues with the pre-determination coordinator, she submitted my information and I'm scheduled to have my surgery on June 9th at 5:30am. It hasn't really sunk in yet that I'm having surgery but I'm sure as the days get closer, I'll definitely freak out. Your prayers are welcome.
