Severe debilitating everyday headaches

My brother had surgery almost 2 years ago. About 10 months ago he started getting severe headaches daily accompanied with nausea, lightheadedness, dizziness, and off balance. He goes to a neurologist and he has tried just about everything. They took away all medications and brought them all back. They have given him Botox treatments. That did not work. He does dip but in the past was adamant about not having it taken away. I recently found out he has increased his usage to 2 1/2 - 3 cans a day. When I researched nicotine poisoning, a lot of his symptoms matched those of nicotine poisoning. Has anyone done dip after their bypass? He has not told his doctor or neurologist how much dip he uses daily. I can't imagine any use of dip is particularly healthy for a person who has had surgery on their stomach. 

There is not a day that goes by headache free. He sits in a dark room all day, chewing tobacco, and severely suffering from a headache. He is presently out on a health leave from his job, but is thinking of applying for disability since he cannot function.

please let me know your thoughts or experiences with this. I am researching and trying to help him find answers, but he doesn't believe it is the dip. If not that, then what?

Desperate for help.
