Hello. My name is "Luis" , I live in Jacksonville Fl, I am 57 years old, well I decide to do my surgery this year I keep gain weight and seeing to me that is enough so I talk to my P.C. "Dra Clara Chaves", and she agree with my desicion, after I read a lots of comments from people here in Obesity Help I decide to do it.
I search for a surgen in my city and I fine "Dr. Robert Cywes" a friend of mine did his surgery with him 3 years ago, I call his office in 7/23/08 they told me to come for a interview with "Danna", she is the insurance coordinator in 8/4/08. I went to see her that day and after she have my imformation about my insurance Humana Gold told me that she will call me back after fine out if they will cover the surgey in about 7 to 10 days also explained about the surgery she was also a patient of Dr. "Cywes" and she loss more than 75 lb, Very nice lady.

8/14/08 "Danna" call me and told me that my insurance will cover the surgery but only I need a referral from my PC.. and also give a appoitment date to see Dr
"Cywes" this date was 9/3/08

9/3/08 in this date also i have appoitment in the morning to see my PC , we talk about the surgery and she told me that go ahead with it that all the referrals that I need she will help me whit that she is a wonderfull lady doctor.
9/3/08.. went to see Dr "Cywes" ..The nurse practitioner "Janet " she check my BP, my Temp, and my weight (463) she ask me few medical questions and told me to wait about 5 minutes to see the Dr. I was so nervous.
well finally the Dr come in and ask me lots a medical questions and explained to me about the surgery and all the things that I have to do to make this work, he was very specific about it went right to the point , he wrote 2 prescriptions, one for my blood work (test) and the other one for my clearence for the Cardiologist. also told me to to get my evaluation from the Psychologist, and to make appoitment to see the Nutritionist..before he left he told me if i gained more weight after that day he will not do the surgery....so no more pounds for me jajaja

9/17/08...I went to see the Psycologist "Ph.D Sari Newman". I was almost 3 hours in her office fill up this questioner (yes and no) questions 350, after I finished she talk about the surgery, and told me that she will fax the results to Dr "Cywes" office..She was very nice and trate me with respect and very professional...Thanks Sari

9/18/08..I went to see the Cardiologist " Dr.Douglas Chapman" in Shands Hospital in Jacksonville Fl. The Dr check my hard and send me to get my EKG right there in the hospital, he was so nice he told the nurse he want that now, no appoitment God Bless Him, if not i have to wait maybe 1 month, well i did it, after he check the results from the xtray he told me that he don't see any problems and wich me good look with my surgery, I thanks him and I was so happy

9/24/08..I went to do my work blood (test), they took lot of blood but I think is necessary.

9/30/08.. I call Dr " Cywes" office to ask what papers they have in there, I talk to "Sandra Valadez " she is a pation representative, very sweet and nice Lady, she told me that they have my medical records from the past 5 years from my PC, and she will going to call my PC to get the results from the Cardiogist and my blood test, also she will contact the Psycologist to she can fax my results..and give me my second visit to see my surgen "Dr Cywes" wich is 10/29/08..Thank You Sandra

10/3/08..I went to see the Nutriotionist " Sandra" She is part of my Dr team very nice lady explained to me the benefits of the surgery but only if I keep good diet and eating good food, doing excercise , will be the only way to succeed with the LAP BAND surgery teach me what I can eat and not to eat and focus in a low carbohydrates diet , Wow this was very educated to me and I am ready to faloow up this diet from now and after the surgery..Thank You "Sandra" good job..
