Question:
What should I write on the form for VLTP? (Federal Employees)

I can remember answering this question once before for someone else on this website under "Other". I even remember EXACTLY what my response was. However, I did not realize that the federal form that I would have to complete on my job would place me in the precarious position of divulging my personal medical condition(s). This particular form is necessary so that I can continue to get paid. Once I have used all of my "leave" hours, I will be in a "non-pay" status. That means I don't get a paycheck. Other employees can "donate" a certain number of their leave time (personal hours/vacation hours to help. On Line #11 of this form, it states "Attach documentation from the physician (or expert showing the diagnosis, prognosis and duration of illness" Also Line #14 states "Description of medical emergency". Now for those of you who are federal employees~~I want to be sure that I don't write the wrong thing on OPM Form 630 (Application to Become a Leave Recipient Under the Voluntary Leave Transfer Program) I don't want to get into any trouble or anything, but I don't want them to know EVERYTHING. I Everybody's business is nobody's business... Don't get me wrong, I'm not concerned about what people think of what I am doing, I just don't want a swarm of people barging into my office telling me how crazy I am and everything else. Also, who's to say that the folks in the Human Resources office won't gossip and spread the word? I know eventually, (post op), I will probably tell, but co-workers don't need to know. Should I call my surgeon's office? I'm at a loss for words now. I could use as many suggestions as possible. ALL that can help I would appreciate it. Hadiyah - I am a federal employee and I am being posed with the same question. I have not yet decided what I am going to put however, I have seen statements that just say the person meets the requirements to participate in the VLTP. While this is very vague, it keeps your secret.

Hi. I have seen these Requests for Annual Leave Donations pass thru my desk alot. And I remember one recently that said "Susie wishes her medical condition remain anonymous..." Or something to that nature. Maybe you could do that. OR -- just say you are having major abdominal surgery.

Oftentimes, I’m so busy reading and responding to others, that I’m unable to get back to my own posts. I try to make time for others just as others have taken time for me both pre and post-operatively.
