Insurance Help

I am hoping to have surgery.  I live in Georgia and currently have Medicaid.  I was wondering if anyone can tell me what the pre-op requirements are for the Medicaid plans Well care, Peach State, and Amerigroup?  Do all these plans require a 6 month supervised diet? I have waited for so long I just want to get it done?  Any surgeon suggestions is appreciated as well.
