Yes, I have Anthem blue cross and I had to lose weight prior to surgery. I also had a $300 co-payment due before my surgery. I was kinda surprised about that. But considering the surgery is very expensive, I was happy just to pay that amount. Best part, I got my own private room in the hospital!

Wow yes I'm hoping I don't have to have 6 months of supervised diet and just have to lose a certain amount of weight instead... I'm hoping I don't have to pay anything before the surgery just after

I didn't have to lose any weight prior with Anthem BCBS.  I had to have only 1 month from start to end of apps but I took nutrition classes for 5 before finding this out.

$300 sound like a bargin to me. While i have coverage thru insurance, it isnt great. Looks like i will hit our out of pocket maximum of $7500.

I am just happy that it looks like i will qualify.

I have Pilgrim Harvard. Had to do about a year of visits but I think that was the center/surgeon I went to. Lost 30 pounds pre-op, me with a nutritionist six times, exercise physiologist three times, two appointments for a psych eval, did a once a week lifestyle changes class for six weeks and my co-pay was $4500. good luck!
