You will need to call the number on the back of the card and ask.

My surgeon and hospital made me pay up front, regardless of which insurance I had. I would ask your surgeon's office how they bill.

You should call the hospital billing office and ask them.  It varies from one hospital to another and your insurance company is not going to know individual hospital policies.

Sometimes you can also set up a billing plan in advance. I did that with my knee surgery. I paid 50% of my estimated co-pay prior to the surgery and then they billed me for the rest once the insurance company evaluated the claim, and I was then able to pay what remained of my portion (20% of allowed costs) in 3 payments.

I was thinking of my deductible which I was billed for later. I guess the co pay would be different.
