Highmark insurance approval

hello! I am in month 4 of my 6 month pre-diet requisite from my insurance. I have heard of people having to jump through hoops to get approval from their insurance. I am just wondering what kind of "hoops" has anyone had to go through? I feel like I am doing everything right by going to my nutrition meetings monthly & getting all my tests done. Has anyone had any experience with this? Also, what can I expect at the physc evaluation? I have that coming up next month.
