You can have your 4 shakes at any time you want to as long as it is before midnight. It sounds as though you are asking if you can have a 5th shake that last day. I'd say no since it is supposed to be 4 per day but the choice is up to you if you feel you need a 5th one. It's better than eating something.

My suggestion is forget the additional and try to get a good full-nights sleep because it's noisy in the hospital and I had trouble staying asleep after surgery.
