STD Coverage and forms? Who, what, when?

I was wondering if anyone has any advice about filling out Short Term Disability Claim Forms with Sun Life or any other firms?  I found an Employee and Attending Physician Statement that needs to be submitted and I called our company insurance support rep to find out when to fill it out, whether my family physician or the surgeon fills it out and all that but they weren't very helpful; they did think that the surgeon should fill it out though.  Has anyone else dealt with this?  If so, would you mind letting me know who you had to fill it out and when you submitted it?  They have restrictions on elective surgery so that is why the rep though having the surgeon fill it out vs. my regular doctor do it would be better, but do the surgeons even do this?  I'm seeing Dr. Huynh.

I'm going to try calling again to see if I can find someone better informed, but it would help if I had a better idea of what is usually done or what I should ask about so hopefully it will be a better discussion with the rep and not so vague.  Any help or advice you can provide is greatly appreciated!  Thanks!

My family doctor filled out my forms for Great West Life, no issues.

I'm at Great West Life and they said that either the surgeon or my family doc could fill out the forms.  I worked with the surgeon's assistant on this - sent her the forms, she got the surgeon to fill them out.  Be prepared for a charge for this - it cost me $100 (might be cheaper through your family doc, but most of them charge as well).

Thanks, would you mind if I asked when you gave them the forms to be filled out?  I'm probably looking at surgery in January or February I would suspect. 

No problem!  I told them about my surgery in the summer, but they said that the insurance company doesn't accept forms any earlier than 2 weeks pre-surgery.  So I didn't even get the forms from them until 2 weeks prior.  The surgeon's office was very accomodating, and it took only a couple of days to get everything sorted out. 

First thing I thought was STD pretty much covers anyone who touches it... who; anyone who touches it, what; IT, When; everytime!
