Age, pants size, and activity level have nothing to do with BMI. It is just a straight up height/weight ratio.
They are asking for those additional things to make ASSUMPTIONS about your body characteristics (a waist to body size ratio, which is extremely flawed based on pants size, since women frequently buy their pants based on the hip, butt, or thigh size rather than waist size and pants size varies WIDELY from one manufacturer to another... I have one pair of size 8, mostly 10s, some 12s and two pairs of 14s!) and to give computerized "advice" on losing weight.
