I finally had the revelation earlier this year that I cannot beat my weight problem with JUST diet and exercise and I needed to take control.  I mean, let's face it, none of us are getting any younger (I'm mid-40's) and I knew that it would just become harder to try the d/e route alone.  I had passing thoughts over the last few years about WLS and checked over insurance coverages several times but it always said basically "not covered".  I didn't realize that BMI and co-morbids played a role in the decision process until my friend had surgery about a year ago and filled me in on her qualifications.  It was at that point that I seriously began to think about it but still thought it was the easy way out.  I hit bottom in January 2009 with an incident one night and decided then and there to start researching.  It was that research that showed me that this is in no way easy and it was, in fact, harder than just the d/e that we've all tried and failed at.  My outlook about changing my eating habits changed almost overnight but putting those habits into practice has been challenging.  I've jumped into this journey with both feet and feel more optimistic about a positive outcome than I have in many years with my previous various attempts at weight loss.  That optimism has helped me change the way I eat and the foods I choose, although I'm not perfect every day.  But my ultimate goal of having surgery and following a program like this has helped me focus more than ever before.
