Insurance Troubles

I got a phone call from the doctor's office today telling me that the insurance denied me. I dont understand at all because i meet all of the requirements and i did everything that i was supposed to do in order to get approved. So the nurse went over everything and re labeled and rechecked everything to make sure it was correct and sent it back in to the insurance. I am so upset because i have been looking forward to this so much and you just assume that you're going to get approved if you meet all the requirements.  I mean there is really nothing else that i can do to get approved. i am just hoping and praying that they will see that i have done everything and i do meet the requirements and approve me. i want this more than anything in the world and if i get denied again then i am gonna be so depressed. this really sucks. it might be stupid of me but i've been crying for the past hour because i'm scared i wont get approved. Did anybody else get denied the first time and then get approved?? i dont know much about the insurance part so could anyone give me some info on why they do this>? Should i get approved the second time around or is it just a hit or miss?
